Recommended steps for responding and reporting a release:

1. Implement emergency response measures, as appropriate
2. Review federal and state release reporting requirements to determine reporting obligations
3. Notify (by phone, to be followed up in writing) applicable agencies to the extent release triggers release reporting requirements
4. Provide a detailed description of incident including,

  • Date and time of release or discovery
  • Specific location of release and media impacted (e.g., water, soil, air)
  • Chemical name(s)
  • Source and cause of the release
  • Amount released
  • Names of exposed individuals
  • Medical assistance provided, if any
  • Actions taken

4. Photograph area implicated by release
5. Notify EHS and legal departments and/or call your Farella Braun + Martel attorney
6. Conduct root cause analysis and identify potential follow up measures
